An exciting opportunity has become available for a Social Worker to join an Older People's Community Mental Health Team within Adult Social Care.

You'll be working with older adults experiencing complex mental health needs, providing person-centred support in the community to promote independence, recovery, and well-being.

Responsibilities:

• Carry out assessments, care planning, and risk management for older adults with mental health needs
• Work closely with community mental health professionals, families, and carers
• Support service users with dementia, depression, psychosis, and other age-related mental health conditions
• Apply relevant legislation including the Care Act, Mental Health Act, and Mental Capacity Act
• Promote strengths-based, recovery-focused practice
• Contribute to safeguarding, crisis intervention, and multi-agency reviews

To be considered for the role, ideally you have:

• BSc in Social Work (or equivalent)
• Social Work England registration
• Experience in adult mental health or older people's services
• A strong understanding of the relevant mental health legislation and frameworks
• Ability to manage a varied caseload and work autonomously
