The Role
The Reablement Enabler shall play a crucial role within the Reablement Service, delivering personalised care and support to adults in accordance with carefully tailored Reablement plans. The primary aim is to assist individuals in regaining or maximising their independence through short-term, intensive support, thereby enabling them to live healthy and autonomous lives within their own homes or chosen environments.
This role demands a compassionate yet enabling approach, working closely alongside health and social care professionals to deliver person-centred care that promotes independence. The service encompasses a broad range of adults, including those with learning disabilities, physical and sensory needs, mental health conditions, and older persons, within community and hospital settings.
The service operates daily from 8am to 10pm, with evolving multi-agency collaborations shaping future service delivery. Flexibility in working patterns, including mornings, evenings, and occasional overnight support, is essential.
Key Responsibilities
Deliver care and support in line with individual Reablement plans, including personal care, domestic tasks, and exercise programmes devised by therapists.
Adopt a strengths-based approach that promotes independence and empowers service users.
Report safeguarding concerns promptly, adhering to statutory safeguarding procedures.
Ensure compliance with the Care Act 2014 and other relevant legislation.
Provide medication support and nutritional assistance as per Support Plans and organisational policies.
Maintain accurate and timely documentation of all visits and service user progress.
Liaise effectively with multidisciplinary teams, carers, families, and community organisations.
Uphold excellent infection control standards and promote health and safety at all times.
Participate actively in meetings, training, supervision, and continuous professional development.
Embrace and demonstrate the organisation's core values of public service, equality, dignity, and empowerment.
Person Specification
Essential Qualifications & Experience:
Level 2 NVQ or QCF Level 3 in relevant discipline.
Experience in adult social care or equivalent domiciliary care setting.
Experience working with diverse populations including ethnic minorities and individuals with varying needs.
Familiarity with strengths-based, person-centred approaches.
Skills and Knowledge:
Awareness of safeguarding legislation and Making Safeguarding Personal principles.
Understanding of key legislation such as the Care Act 2014 and Mental Capacity Act 2007.
Ability to work within multi-disciplinary teams and build effective partnerships.
Proficient in accurate record-keeping and use of IT systems.
Competent in supporting personal care, medication, and mobility tasks safely.
Strong communication skills, both verbal and written.
Willingness and ability to travel within the service area and work flexible hours.
