Full-time position (37 hours), but open to considering 29.6 hours (4 days) for suitable candidates.
Role involves working in accordance with the Social Service and Well Being Act.
Responsibilities include conducting integrated assessments, mental capacity assessments, reviews, providing support to unqualified staff, and offering community/hospital support.
Working with adults aged 18 and over with physical disability and/or mental health diagnosis.
Fast-paced work environment requiring effective diary management skills.
Candidates must be able to provide adequate references at the interview stage.
Seeking a valuable addition to the team.
Registration in Wales is a must for this role.
Prior adult experience and readiness to handle cases immediately are essential.
Must have references available for review if called for an interview.
Ability to answer basic questions during the interview, including knowledge of relevant legislation, is required.
Location: Mardy Park resource centre, Monday to Friday.
