Rate: £50-£55 per hour
Flights Paid | Accommodation roughly £30 per day

Are you an experienced Senior Social Worker looking for a new challenge? We have an exciting opportunity for you to join a Hospital Discharge Team on the Isle of Man, helping patients transition safely from hospital to community care.

The Role:

• Facilitating safe and effective hospital discharges

• Conducting assessments and coordinating care plans

• Working within a multi-disciplinary team to ensure the best outcomes

• Supporting patients and their families through the discharge process

• Managing complex cases and safeguarding responsibilities

Requirements:

• Degree in Social Work & Social Work England registration
