Job Description - Social Worker




6 months contract with a Local Authority



Job Summary:



• A dedicated and qualified Transition Social Worker is required to join an Adult Social Care team on a full-time basis.



• This role focuses on supporting young people transitioning from Children’s Services into Adult Services, ensuring that their needs are assessed, planned for, and appropriately supported in accordance with the Care Act 2014 and other statutory responsibilities.



• The post holder will manage a caseload of customers with varying and often complex needs, working collaboratively with families, carers, education, health services, and other partners to promote independence, safety, and wellbeing.







Key Duties/Accountabilities (Sample):



• Complete Care Act Assessments, support planning, and reviews for young adults transitioning into Adult Social Care.



• Undertake safeguarding enquiries and escalate where necessary in line with Making Safeguarding Personal principles.



• Liaise effectively with Children’s Services, Health, Education, Housing, and other stakeholders to ensure coordinated support.



• Use eligibility criteria to differentiate needs and determine appropriate support plans, including the use of Resource Allocation Systems (RAS).



• Contribute to risk assessments and identify timely interventions for urgent needs.



• Commission services that reflect best value and outcome-focused delivery.



• Maintain accurate case records using ICT systems and ensure compliance with data protection policies.



• Provide guidance and occasional supervision to junior or non-qualified staff.



• Work in line with legal frameworks including the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.



• Participate in multi-agency meetings and contribute to joint planning for service users.



• Support performance management through personal development, reflective practice, and continuous learning.







Skills/Experience:



• Experience working with young people transitioning into adult services.



• Sound understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.



• Ability to manage a complex caseload and respond to changing priorities.



• Skilled in undertaking assessments, developing outcome-focused care/support plans, and arranging appropriate services.



• Strong communication skills – both written and verbal – for liaising with internal teams and external stakeholders.



• Competent IT skills, ideally with experience using Liquidlogic IAS or similar case management systems.



• Demonstrated ability to work autonomously while maintaining team collaboration.



• Ability to manage risk effectively, including emergency responses and crisis management.







Additional Information:



• Maintain professional registration and Continuing Professional Development (CPD).



• Degree in Social Work and registration with Social Work England (essential).



• Working Hours: 36 hours per week (Monday–Friday, 09:00–17:00).



• Location: Office-based with hybrid/fieldwork flexibility (Guildhall 2).
