What you will do:

• Conduct risk assessments and promote risk enablement, supporting people with complex mental health needs.
• Build and maintain effective working relationships with colleagues and professional partners.
• Undertake both care coordination and Local Authority functions, using the Care Act to assess care and support needs.
• Develop creative support plans that enable independence and tap into natural support networks.
• Work closely with other agencies and organisations to ensure comprehensive care and support.
• Provide mentorship and support to newly qualified staff and foster a culture of continuous improvement.
