Join a dynamic Community Mental Health Team (CMHT) in West London as a Band 6 Social Worker. In this vital role, you'll work closely with individuals with complex mental health needs to support recovery and stability within the community. Your key responsibilities will include:

? Managing a caseload of adults with mental health needs.
? Conducting assessments, care planning, and delivering recovery-oriented interventions.
? Collaborating with multidisciplinary teams, families, and external agencies.
? Providing safeguarding interventions and comprehensive risk assessments.
? Supporting service users in accessing housing, benefits, and other social care support.
? Ensuring all work complies with current legislation and best practice guidelines.

To be successful in this role, you will need:

? A current and valid Social Work England registration.
? Experience working within a Community Mental Health setting.
? Strong assessment, safeguarding, and care coordination skills.
? Excellent communication and interpersonal abilities.
? The ability to work independently and collaboratively.
? A valid full UK driver’s license (preferred but not essential).

This is a full-time role, Monday to Friday, 9 AM - 5 PM. The contract is ongoing, with potential for extension depending on service needs and performance.
