What you will do:

• Conduct initial assessments of adults needing support and promptly manage a caseload of two cases per week
• Collaborate with multidisciplinary teams to create tailored support plans
• Provide advice and guidance to adults, their families, and carers to empower and support them in decision-making
• Stay up to date with legislation and best practices in adult social care
• Ensure accurate and timely recording of all actions and decisions
• Actively contribute to team meetings and case reviews
• Visit clients in their homes or other settings across Wokingham, so having access to a car is essential
