About the Role:
We are seeking an experienced and motivated Continuing Health Care (CHC) Social Workerto join a dynamic team based in Leeds. This is an exciting opportunity to make a meaningful impact while enjoying the flexibility of hybrid working.

Key Responsibilities:

• Undertake Continuing Health Care assessments in line with the National Framework

• Work collaboratively with NHS professionals, families, and other key stakeholders

• Support clients through the CHC process, including appeals and reviews

• Complete strength-based assessments and develop person-centred care plans

• Maintain accurate records and ensure compliance with local and national policies

Requirements:

• Qualified Social Worker with current SWE registration

• Proven experience in Continuing Health Care and hospital discharge planning
