What You Will Do

• Patient Assessments – Conduct comprehensive assessments of patients' social, emotional, and environmental needs to facilitate safe, timely discharges.
• Care Planning – Develop individualized care plans in collaboration with patients, families, and healthcare professionals.
• Discharge Coordination – Implement and manage discharge plans, ensuring appropriate community support and follow-up care.
• Multi-Agency Collaboration – Liaise with healthcare providers, social care agencies, and community resources to provide integrated care.
• Safeguarding Responsibilities – Identify and address safeguarding concerns, protecting vulnerable adults in line with legal frameworks.
• Advocacy – Champion patients' rights and ensure access to resources, while maintaining professional and ethical standards.
• Record-Keeping – Maintain accurate, timely records of assessments, interventions, and care plans in compliance with legal and organizational standards.
• Legal Compliance – Ensure all practices align with the Care Act, Mental Capacity Act, and other relevant statutory guidelines.
• Team Collaboration – Participate in case reviews, team meetings, and service evaluations to support continuous improvement.
