What you will do:

• Conduct comprehensive assessments of individuals with mental health conditions, evaluating their physical, emotional, and social needs.
• Develop, implement, and review care plans in collaboration with service users and their families.
• Support individuals to access relevant mental health services, resources, and interventions.
• Provide case management and advocacy to ensure individuals receive necessary services and support.
• Liaise with healthcare providers, housing, and community agencies to coordinate care and services.
• Participate in team meetings, supervision, and continuing professional development activities.
• Maintain accurate and timely records of assessments, care plans, and interactions with service users, adhering to safeguarding protocols and confidentiality guidelines.
• Provide expert social work support in crisis situations, ensuring effective risk management and decision-making.
• Promote social inclusion, offering guidance on housing, employment, and social integration.
