Social Worker - Prevention & Wellbeing

Halton Borough Council are looking for an experienced and passionate Adult Social Worker to join their Prevention and Wellbeing Team. This is a fantastic opportunity to be part of a proactive and supportive service that focuses on early intervention and enabling people to live healthier, more independent lives.

Key Responsibilities:

• Conduct strengths-based assessments and person-centred care planning.
• Work collaboratively with health professionals, voluntary sector partners, and families.
• Provide advice, information, and support to reduce risk and promote independence.
• Ensure Care Act 2014 compliance and promote safeguarding principles.
• Support timely discharges from hospital and reduce avoidable admissions.

Requirements:

• Social Work qualification (BA/MA/DipSW or equivalent).
• Current registration with Social Work England.
• Experience in adult social care, particularly in preventative or early intervention work.
• Excellent knowledge of the Care Act and safeguarding adults.
