Job Description - Supported Pathway Officer

A new opportunity has arisen for a Supported Pathways Officer to join the Resident Services division at Southwark Council. This role is dedicated to assisting residents in securing their next stage of housing and providing resettlement support to help them sustain their new home. The officer will also offer floating support for six months and ensure that residents are connected to the necessary support services thereafter.

Responsibilities:
  • Provide key-working, risk assessment, and support planning to a caseload of approximately ten clients.

  • Assist residents in maintaining their accommodation and engaging with services that support their well-being.

  • Deliver a trauma-informed approach to key working, helping residents take preventative actions against homelessness.

  • Monitor and assess risks to residents and colleagues while ensuring compliance with all council policies and procedures.

  • Support residents in accessing independent living services and sustaining their tenancies.

  • Advocate for residents by liaising with social services, health agencies, and welfare support services.

  • Organise and participate in activities to promote social engagement and confidence-building.

  • Manage rent accounts, assist with benefit issues, and help maximise residents' income.

  • Conduct estate inspections and oversee maintenance services to ensure safe and habitable living conditions.

  • Handle void management, resident bookings, and risk assessments for new arrivals.

  • Enforce tenancy conditions, addressing anti-social behaviour, and where necessary, coordinate legal action.

Requirements:
  • Experience working with homeless individuals or those with complex support needs.

  • Strong knowledge of the Care Act, housing legislation, and welfare benefits.

  • Understanding of Adverse Childhood Experiences and trauma-informed care.

  • Excellent risk assessment and support planning skills.

  • Ability to work collaboratively with internal teams and external agencies.

  • Strong organisational skills and the ability to work independently.

  • Commitment to equality, diversity, and safeguarding policies.
