In this crucial role, you'll be responsible for overseeing the annual review process for adults with care and support needs, ensuring that individuals receive the right level of support to maintain their independence and well-being. You'll lead a committed team, providing guidance, supervision, and support to deliver high-quality, person-centered outcomes.

Key Responsibilities:

• Lead and manage the Adult Annual Reviewing Team, promoting a positive and supportive work environment.
• Oversee the effective and timely completion of annual reviews for adults with care and support plans.
• Ensure reviews are person-centered, outcome-focused, and compliant with relevant legislation and local policies.
• Provide professional supervision, guidance, and development opportunities to team members.
• Work collaboratively with other teams, agencies, and stakeholders to ensure integrated and holistic support.
• Manage team performance, caseloads, and resources effectively.
• Contribute to the ongoing development and improvement of services within Adult Social Care.
