Job Description - Wellbeing Officer (Carers)

Wellbeing Officer

We are looking to recruit a Wellbeing Officer for the Neighbourhoods and Wellbeing Service within City of Doncaster. The successful candidates will provide bespoke support to individuals, face-to-face and in their own homes or communities. Assessing needs, completing comprehensive assessments and producing thorough support plans to help improve people's health and wellbeing and maintain their independence, to enable them to remain safe and independent within their own homes and communities.

Job Details:

  • 7 hours per day, 5 days per week.
  • Contract type: Temporary, minimum of 3 months with the potential to become permanent.
  • Full UK driving licence and vehicle required.
  • Hybrid role which may also consist of lone working.
Candidate Profile:

We are looking for a positive, proactive and flexible person who is hardworking and conscientious with an energy to support people and think creatively. The ideal candidate will be well-organised, with excellent communication and relationship skills and the ability to communicate sensitive information in an appropriate way. You should also be confident, non-judgemental and resilient. Professional and/or personal experience and understanding of health, care and the community are essential elements within this role

Duties and Responsibilities

Core responsibilities and duties will include:

  • Frontline delivery of an "all age person centred" service in a locality, to help people to lead healthy, connected and active lifestyles, which allow them to remain living independently in their homes for longer. Ensuring that individuals have choice and control in how their support is provided
  • Customer facing delivery of early intervention and prevention solutions to reduce escalation to costly services
  • Work with a range of partners, residents and services to promote and influence on, early intervention and prevention, wellbeing, Community Led support and community capacity building. To develop an understanding of the needs of local people which will enable communities to do more for themselves and develop their own solutions.
  • To develop professionally and have the ability to undertake trusted assessor roles around physical home improvements, fall prevention, carers and other areas of wellbeing work as service develops in improving low level support across Health and Care.
  • Identify gaps in community provision and customer needs, with ability to negotiate with and influence communities, council services and partner organisations, to ensure that options are developed and supported, including assistance with applying for and obtaining funding opportunities
  • Take a holistic approach to both case management and case progression to ensure positive outcomes for those engaged within the Wellbeing Service
  • Progress individual casework to improve people’s Wellbeing, including physical health, mental health, loneliness, finances, safeguarding, environment and housing.

Specialist Skills and Knowledge

Essential skills, knowledge and experience required to fulfil the role include, but are not limited to:

  • Experience of working in the community, in a health and/or social care setting.
  • Experience of working in a public sector and associated statutory requirements for adult health and/or social care setting with a good understanding of local and national health and wellbeing strategies including; legal requirements, Doncaster’s health needs, The Care Act and Early Intervention and Prevention work.
  • An understanding of the health and wellbeing needs of potential service users and carers with knowledge of vulnerable groups and associated issues.
  • Experience of stimulating community developments to impact on their health and wellbeing
