o West Berkshire have an exciting position for a motivated, conscientious, and experienced Health care assistant within Newbury Community Nursing services.o The Community Nursing service offers care to patients in their own homes, 7 days a week, 08.30 - 19.00, inclusive of Bank Holidays.The post holder will be able to demonstrate the competencies required to perform at this level, role model the Trust's core values as well as exhibit the behaviours required as part of those values.o The successful candidate will work across health and social care teams, playing an active role as part of a multi disciplinary team. He/she will provide planned care with proximal supervision from a registered practitioner to deliver a seamless service to patients.o You will be required to proactively work with other team members embracing case management and the long-term conditions agenda thus preventing avoidable hospital admissions and facilitating appropriate earlier discharges into the community. o He/she will be proficient and competent to work within Community Nursing teams as well as supporting and working alongside our spoke services such as community matrons, as deemed appropriate to their specific role, and the triage service. He/she will work within defined protocols and be able to take appropriate action to deliver planned care.Interviews to be held 26th April 2023
