Job Description - Family History and MDT co-ordinator


· To work with the assistant service manager, booking co-ordinators, nurses and doctors to deliver high quality administrative services to the St Mark’s Centre for Familial Intestinal Cancer. · Work with the nurse practitioners to input family history data onto the centre’s database and produce family pedigrees as required. · Track at risk family members and ensure they are contacted and offered appropriate genetic testing, using the FileMaker database. · Contact attending patients to discuss family history (as identified by the clinical team) so that any gaps can be resolved, and family history updated. · To be responsible for the coordination of the unit MDT meetings, including preparation of agenda and provision of minutes. · Cross-site travel may be required. The post holder is required to work alongside and under the supervision of the assistant service manager and nurse consultant and ensure regular communication with the nurse practitioners, lead clinicians and support services. i.e., radiology, pathology, out-patients, using email, telephone, face to face and in writing as required. The post holder must communicate to all with due regard to disabilities. The post holder must be competent in word processing and basic spreadsheet skills. The post holder is required to input data, daily, regarding the patient journey into the FileMaker database. They will be required to analyse the data to assess if targets may be at risk of being breached, taking actions to eliminate a breach in targets. The post-holder will participate in a weekly tracking meeting (at risk family members), highlighting any issues with patient pathways. To assist the clinical team in ensuring positive patient experience is central to the patient pathway. Although there is no direct responsibility for patients, the post holder is required to ensure that patients are given appointments within target times and information regarding scans and other tests are relayed to them correctly. If they are required to communicate with patients regarding their appointments, this must be done in a professional and sensitive manner. • Central Middlesex Hospital: our planned care site, hosting a range of surgical and outpatient services and collocated with an urgent care centre. We are a university teaching NHS trust, in recognition of the important role we play in training clinicians of the future and bringing the benefits of research to the public. To work with the assistant service manager, booking co-ordinators, nurses and doctors to deliver high quality administrative services to the St Mark’s Centre for Familial Intestinal Cancer. · To be responsible for providing co-ordination and administrative support for St Mark’s Centre for Familial Intestinal Cancer MDTs. · To add family history information to the database, producing electronic family pedigrees and ensuring at risk relatives are alerted (in conjunction with the centre’s nurse practitioners). · Coordination of unit MDT meetings. · To support the assistant service manager and booking coordinators to ensure all centre administrative process are delivered including data and evidence collection for annual review and relevant national audits.
