Our professional responsibility is to reduce the risk of infection to others as far as possible. Getting vaccinated against diseases that can be passed person-to-person in healthcare settings is part of this responsibility. The Trust will provide advice, guidance and support on vaccinations and access to vaccinations where requested. An exciting opportunity has arisen to join the Mother and Baby Unit Outreach Service in Somerset for 30 hours per week (0.The post holder will be part of the Mother & Baby Unit Outreach Service, we are keen to invite applications from those working within mental health settings and encourage applications from RMNs/AHP's. The Outreach Practitioner will work with admission vulnerable women from the third trimester to 12 months postnatal. This involves delivering a robust care pathway for women in the community, seeing them prior, during and post hospital admission, for up to four weeks post discharge, ensuring high quality co-ordinated care at all times. The role involves working across Somerset in the community, alongside Somerset Outreach colleagues, with some lone working. There will be some expectation, where appropriate, to travel to Jasmine Lodge to maintain contact with patients who may be admitted, or to attend MDT when this is not possible via Teams. The role involves some home working and some provision of cover for the duty service on a rota basis, this operates 1000 - 1600 daily. The Mother and Baby Unit (MBU) Outreach Service provides assessment and intensive support/treatment of women who would otherwise be admitted to our MBU - Jasmine Lodge, and facilitates early discharge from the MBU in order to prevent relapse and re-admission. Our MBU Outreach Service offers assessment and treatment of the mothers mental illness, whilst ensuring the developing relationship with their baby, focusing on bond and attachment. The service works with mums who are experiencing a severe episode of mental illness. This includes mums who might benefit from an admission to our Mother and Baby Unit (MBU) - Jasmine Lodge. Jasmine Lodge is located in Exeter, and the practitioners from our Outreach Service are based across Devon, Cornwall and Somerset. They work collaboratively with other local health services and community perinatal teams to ensure mums and their families receive joined-up care, when and where they need it. This is a maternity cover post, covering one year. We provide mental health, learning disability and neurodiversity services, as well as a range of specialist & secure services for the wider south west region & nationally. The role of the Outreach Practitioner involves working with women in the perinatal period (32 weeks pregnant to 12 months postnatally) who are MBU admission vulnerable, with a significant psychiatric diagnosis or presentation. The MBU Outreach Service provides assessment and intensive intervention, support and treatment of women who would otherwise be admitted to the Mother and Baby Unit for inpatient psychiatric treatment. The service works with mums who are experiencing a severe episode of mental illness. This involves timely assessment of presentation, robust formulation, risk assessment and care planning. The Outreach Practitioner is part of a highly skilled wider MDT, however a significant element of this role is lone working, within the community. Therefore a strong ability to assess and manage risk related to mental health and safeguarding, autonomously, is required. The service works closely with key internal and external personnel including midwifery, acute medical staffing, safeguarding, housing, voluntary and non-statutory agencies and a wider health and social Care community. Ensure close liaison with primary health care staff and commissioning officers regarding referral criteria into the services and quality standards and ensure targets from the National Framework are met. Investigate complaints, grievances and performance issues taking remedial action as necessary. Actively participate in multi-agency forums to ensure the identification of need and the development and delivery of integrated local services in line with local need. Ensure that activity is delivered in accordance with agreed quality standards and to service level agreements and in line with relevant national guidance. Ensure robust systems are in place to monitor the clinical practice of team members in line with CQC standards Ensure effective care pathways are in place for transition between mental health and other services eg Acute Medical, CAMHS, Child Protection, Learning Disability. Ensure the availability of choice that meets the needs of the local population. Work closely with other teams with the Trust and the Voluntary, Statutory and independent sectors to ensure effective discharge planning which incorporates implementation of key success criteria for NICE guidance eg NICE 192 Antenatal Postnatal Mental Health, CPA, Information Sharing etc. Implement a formal and active system of caseload review, caseload management and pro-active care management that will enable achievement of local and national targets. Participate in safeguarding adults and children to level 4 as set out in safeguarding children and young people: roles and competencies for health care staff. Ensure the development of local expertise, ongoing service improvement and professional monitoring of standards. Identify and implement best practice in the delivery of mental health services within the team and disseminate these throughout the service. Initiate service developments and projects. Plan provision of local services in conjunction with the service and other managers. Ensure risk registers and Quality Improvement Plans are embedded within team structures and remain current. Participate in the development, implementation and evaluation of multi-disciplinary client records. Monitor standards and ensure Trust policy compliance including activity data, manual and electronic record keeping and implementation of CPA. Ensure robust risk management systems are in place: Have an on-going awareness of high clinical risk situations within the relevant teams and advise clinicians on actions to be taken to respond to risk. Attend other meetings relating to risk management where other agencies have a statutory role as chair. Practice in a way that actively minimises dependency and promotes recovery. Communicate information in a way that makes it relevant and understandable for all including service users and carers, working to the principles of the functions policy implementation guidelines and practice in line with the standards and values set out in them. RMN or equivalent ( Social Worker/OT) with significant experience of working within a mental health setting. Substantial experience of working at a senior clinical level. Experience of robust assessment and management of risk. Experience of working within mental health team such as CMHT, HTT, PLS, Assessment Servicer, First Response, or an inpatient setting. An awareness of the principles of care for perinatal mental health intervention. Extensive experience of managing a busy caseload of clients with a range of mental health problems. Working within a community mental health setting in either the statutory or voluntary sector. Able to produce well written reports and computer literacy. Experience of perinatal mental health within the community or an MBU. Experience of working with high risk, acute mental illness. Post graduate qualification relevant to specialism, or commitment to work towards. Training to level 3 in Safeguarding adults, children and young people: roles and competencies for Health Care staff, or commitment to work towards. Experience of working within crisis and risk management frameworks.
