The Role

We're recruiting experienced Discharge to Assess Social Workers to join a busy and supportive team focused on delivering safe, timely hospital discharges.

Working closely with health professionals, therapists, ward staff, and community partners, you'll complete strengths-based assessments following discharge, coordinate care and support, and ensure individuals receive the right services to maximise independence and reduce unnecessary readmissions.

You'll be responsible for:
 
Completing Care Act assessments within the Discharge to Assess pathway.
Supporting safe and timely discharges from acute hospital settings.
Undertaking Mental Capacity Assessments and Best Interest decisions where required.
Coordinating care packages and community-based support.
About You

To be considered, you'll need:
 
A recognised Social Work qualification.
Current Social Work England registration.
Experience working within Hospital Discharge, Discharge to Assess, or Adult Community Services.
A sound understanding of the Care Act, Mental Capacity Act, and strengths-based practice.
Experience using Mosaic or a similar case management system is desirable.
