Job Description - Adults Social Worker - Learning Disabilities and Autism
Our client Telford and Wrekin Council is looking for a Qualified Social Worker to join their Adults with Learning Disabilities and Autism team.
About the Role
As a Qualified Social Worker, you will manage a caseload of adults, delivering strengths-based assessments, support planning, safeguarding interventions, and reviews to promote independence, wellbeing, and positive outcomes.
Key Responsibilities
Manage a professional caseload, completing assessments, support plans, reviews, and risk assessments. Apply strengths-based and asset-based approaches to support individuals and families. Promote safeguarding and undertake investigations where required. Work collaboratively with statutory, voluntary, and community partners to achieve agreed outcomes. Attend and contribute to peer reviews, reflective practice sessions, and team discussions. Maintain accurate case records, reports, and documentation. Provide professional advice and guidance to colleagues and support less experienced staff and students. Undertake duty responsibilities as part of the team rota. Promote equality, dignity, choice, and person-centred practice.Additional Requirements / Expectations
Qualified Social Worker registered with Social Work England. Knowledge of Adult Social Care legislation and best practice, including the Care Act 2014 and Mental Capacity Act 2005. Ability to make evidence-based decisions and effectively manage risk. Commitment to continuous professional development and reflective practice.
