Assessment and Enablement Officer – Adult Social Care

Location: Colindale, London
Rate: £30.81 per hour
Contract: Temporary / Interim

We are seeking an experienced Assessment and Enablement Officer to join an Adult Social Care service in Colindale.

You will undertake strengths-based assessments and reviews for adults with learning disabilities, autism, mental health needs, physical disabilities and older adults, ensuring care and support arrangements remain person-centred, outcome-focused and promote independence.

Key Responsibilities

* Undertake Adult Social Care assessments, reviews and risk assessments.

* Review care packages across supported living, home care, residential care, day services and community support.

* Promote independence, wellbeing, choice and control through strengths-based practice.

* Work collaboratively with residents, families, carers, providers and partner agencies.

* Apply the Care Act 2014, Mental Capacity Act 2005 and safeguarding principles.

* Identify opportunities to improve outcomes and ensure support is appropriate and proportionate.

* Produce clear, accurate and evidence-based assessments and recommendations.

About You

You will have:

* Experience working in Adult Social Care and undertaking assessments and reviews.

* Strong knowledge of the Care Act, Mental Capacity Act and safeguarding.

* Experience supporting adults with complex and diverse needs.

* Excellent communication, assessment and professional judgement skills.

* The ability to manage a varied workload and make evidence-based recommendations.

* A strong commitment to promoting independence and person-centred outcomes.

Rate: £30.81 per hour | Location: Colindale, London

Mandatory: Must have a UK Passport or a Full Right to Work in the UK
