Job Description - Assistant Practitioner

Assistant Practitioner

37.5 hours per week

Salary: Up to £27,000

We are looking for a compassionate and experienced Assistant Practitioner with a Level 5 Foundation Degree in Assistant Practitioner (Healthcare) to join our client's team. The successful candidate will have experience working in healthcare and preferably in a specialist service such as Neuro or Palliative care.

What We’re Looking For

  • Level 5 Foundation Degree in Assistant Practitioner (Healthcare)

  • Previous experience in a healthcare setting, ideally in a specialist service such as Neuro or Palliative care

  • Familiarity with relevant legislation and its impact on current healthcare practice

  • Ability to work effectively within a multi-disciplinary team

  • Excellent verbal and written communication skills

  • Strong interpersonal skills and the ability to build rapport with clients and colleagues

  • Capability to respond to individuals in distress with empathy

Responsibilities

  • Support the assessment, planning, delivery, and evaluation of individualised care within competency or under guidance, working collaboratively with the team.

  • Maintain high care standards by adhering to guidelines, challenging poor practices, and supporting staff development.

  • Ensure continuity of care through clear communication and timely reporting of any changes in service users' conditions.

  • Promote choice, encourage feedback, and support service users in having their voices heard in care decisions.

  • Foster a culture of safety by enabling staff to challenge unsafe practices through appropriate channels.

  • Contribute to a safe and efficient service, maintaining confidentiality and taking on reasonable additional tasks as needed.
