Job Description - Care Coordinator

Job Types: Full-time, Permanent

Salary: £26 000 per year

Hours: 37.5 hours per week

Location: Bristol

We are recruiting on behalf of our client for an organised and proactive Care Coordinator to support the safe, compliant, and high-quality delivery of community-based care services. This is a pivotal operational position, well suited to an experienced senior carer or care professional ready to take on greater responsibility within service coordination, quality oversight, and regulatory compliance.

About the Role

As a Care Coordinator, you will oversee the day-to-day delivery of care across the community, ensuring services remain responsive, person-centred, and fully aligned with regulatory requirements. You will provide first-line guidance and support to care staff, while working closely with the Service Lead to ensure smooth workforce planning and consistent service standards.

You will take responsibility for allocated quality assurance tasks, including medication audits, spot checks, equipment reviews, and documentation monitoring, ensuring the service remains inspection-ready at all times. The role also involves supporting rota management, safeguarding procedures, care plan updates, and providing emergency response cover when required.

This position blends operational coordination with quality monitoring and business continuity duties. The salary reflects accountability for audits, rota support, emergency response responsibilities, and participation in the out-of-hours on-call rota.

Key Responsibilities

* Provide daily coordination and oversight of community-based care delivery, ensuring continuity and quality of service.

* Support the Service Lead with operational matters, assisting with staffing gaps, service queries, and day-to-day challenges.

* Carry out quality assurance activities including medication audits, spot checks, and compliance reviews to maintain regulatory readiness.

* Contribute to safeguarding processes, incident management, and risk mitigation in line with current legislation and best practice.

* Assist in reviewing and updating care plans, risk assessments, and service user documentation to promote safe and effective care.

* Support rota planning, manage last-minute changes, and respond to urgent cover needs, including delivering care where necessary.

* Participate in the out-of-hours on-call rota, making informed decisions to maintain safe and uninterrupted service provision.

* Maintain accurate records and contribute to continuous improvement initiatives across the service.

About You

* A minimum of one year’s experience within domiciliary or community-based care services, ideally within a senior or coordination role.

* Working towards, or willing to work towards, a Level 3 qualification in Health and Social Care.

* Sound knowledge of CQC standards, safeguarding frameworks, and quality monitoring processes.

* Experience completing audits, staff observations, and service checks.

* Confident supporting and guiding frontline staff while maintaining accountability and professional standards.

* Able to prioritise effectively, manage competing demands, and remain calm under pressure.

* Strong organisational, communication, and decision-making skills.

* Full UK driving licence and willingness to travel within the community as required.

* A compassionate and values-driven professional committed to delivering safe, person-centred care.

Benefits

* Competitive salary package reflective of responsibilities.

* 28 days annual leave to support work-life balance.

* Ongoing training and structured career development opportunities.

* Supportive and collaborative leadership team.

* Access to company sick pay scheme.

* Enrolment into a contributory workplace pension.

* Regular team-building activities and company-hosted events.
