Care Co-Ordinator

An excellent opportunity has become available for an experienced Care Coordinator to join a well-established, privately owned domiciliary care provider in Eastbourne.

Working alongside the Registered Manager, you will play a key role in ensuring the delivery of safe, high-quality, person-centred care. This is a varied role that includes managing staff and client rotas, coordinating care packages, completing client assessments and person-centred care plans, and maintaining compliance with CQC regulations. You will also provide support within the community when required and participate in the on-call rota.

This opportunity would suit someone with previous experience as a Care Coordinator or in a similar role within domiciliary care who enjoys building relationships, supporting care teams and delivering excellent customer service.

Requirements

* Previous experience as a Care Coordinator or similar within domiciliary care.

* Good understanding of CQC regulations and person-centred care planning.

* Experience completing client assessments and care plans.

* Strong communication, organisational and IT skills.

* Willingness to undertake care visits when required.

Responsibilities

* Supporting the Registered Manager with the day-to-day coordination of the service.

* Managing staff and client rotas to ensure the effective delivery of care.

* Completing client assessments and person-centred care plans.

* Liaising with clients, families, care staff and healthcare professionals.

* Ensuring care is delivered in line with CQC requirements.

* Participating in the on-call rota.

* £29,000 - £30,000 per annum.
