💙 About the Role

As a Care Coordinator, you’ll be responsible for coordinating care packages, managing staff rotas, supporting care workers, and ensuring service users receive the right care at the right time.

🔑 Key Responsibilities

* Coordinate and manage daily care schedules and staff rotas.

* Allocate care workers to service users, ensuring continuity of care wherever possible.

* Respond to changes in service users’ needs and staffing requirements.

* Maintain accurate care records, schedules and documentation.

* Liaise with service users, families, care staff and healthcare professionals.

* Monitor attendance, punctuality and service delivery.

* Support recruitment, induction and ongoing communication with care staff.

* Ensure care services meet safeguarding requirements and relevant quality standards.

* Previous experience in care coordination or a similar role within the care sector.

* Excellent organisational and time-management skills.

* Strong communication and problem-solving abilities.

* The ability to work well under pressure in a fast-paced environment.
