Position: Care Manager

We are seeking a highly motivated and compassionate individual to join our team as a Care Manager. As a Care Manager, you will play a crucial role in overseeing the care and well-being of our clients. This is a full-time, permanent position with the potential for growth within our company.

Responsibilities:

- Develop and implement care plans for clients based on their individual needs and preferences

- Coordinate and monitor the delivery of care services by our team of caregivers

- Conduct regular assessments and evaluations of clients' physical, emotional, and mental well-being

- Collaborate with healthcare professionals, family members, and other stakeholders to ensure the best possible care for clients

- Maintain accurate and up-to-date records of clients' care plans and progress

- Provide support and guidance to caregivers, including training and supervision

- Advocate for clients' needs and rights, ensuring that they receive the highest quality of care

- Monitor and manage clients' budgets and assist with financial planning, if needed

- Stay up-to-date with industry trends and best practices in care management

Requirements:

- Bachelor's degree in social work, nursing, or a related field

- Minimum of 2 years of experience in care management or a related field

- Excellent communication and interpersonal skills

- Strong organizational and time-management skills

- Ability to work independently and as part of a team

- Compassionate and empathetic nature

- Knowledge of healthcare systems and resources

- Familiarity with medical terminology and conditions

- Proficient in Microsoft Office and electronic medical record systems

- Must pass a background check and drug test
