Job Description - Career opportunities Health and Wellbeing Connector


Team: Active Lifestyles Team


Job Family: Supporting - CBG8
Salary: £31,022 - £33,851 per annum pro rata
Hours: 37 hours per week


Location: You will be expected to attend a CBC office location or CBC Library, as often as needed to carry out your role, which will be no less than once per month.


Contract: Hybrid - Fixed-term - 2 years.


 


Closing Date: 5th July 2026

About the Role


 


This is an exciting opportunity to join a public health-funded initiative tackling social isolation and health inequalities in areas of deprivation across Central Bedfordshire. As a Health and Wellbeing Connector, you will play a vital role in early intervention and prevention, supporting residents experiencing loneliness, low confidence, inactivity and digital exclusion.


Working within local libraries as trusted community spaces, you will empower residents to identify their needs, build confidence and take small, positive steps towards improved wellbeing through personalised support and community-based activity.


 


About the Service / Team


You will be part of the Active Lifestyles Team, within Leisure, Libraries and Countryside, which delivers a wide range of wellbeing opportunities aligned to the Council’s Leisure Strategy and wider health objectives.


The role works in close partnership with Library Engagement Officers and community partners, embedding activities into everyday library services and strengthening libraries as inclusive hubs for social connection, digital inclusion and wellbeing.


You will engage with a range of stakeholders including residents, community organisations and health professionals to ensure a coordinated and person-centred approach.


 


Working Arrangements


This is a hybrid role, primarily based within Central Bedfordshire libraries and community settings.



  • Regular travel across Central Bedfordshire is required

  • Face-to-face delivery of sessions and resident support is a core part of the role

  • Standard working pattern is Monday to Friday, with flexibility to meet service needs. This may include evening and weekends


 


Key Skills and Experience


We are looking for someone who is passionate about community wellbeing and making a difference. You will bring:



  • Experience of working directly with individuals or communities to support health and wellbeing, particularly those who are vulnerable or isolated.

  • Experience delivering group activities or community-based sessions

  • Ability to engage hard-to-reach or underrepresented groups.

  • Strong communication and relationship-building skills

  • Confidence in delivering person-centred support and behaviour change approaches

  • Good organisational skills, including monitoring outcomes and recording impact


 


Qualifications:



  • Level 2 qualification (or equivalent experience) in a relevant field such as health, wellbeing, community development, social care or physical activity.


 


What is the Role? (Responsibilities & Objectives)


As a Health and Wellbeing Connector, you will:



  • Identify and engage residents at risk of social isolation

  • Deliver and support social, digital and wellbeing activities within libraries and communities

  • Provide person-centred wellbeing conversations, using motivational approaches to build confidence and resilience

  • Support residents to access appropriate services and navigate local pathways

  • Work collaboratively with partners to create inclusive, welcoming community environments

  • Monitor and evaluate outcomes, including improvements in loneliness, wellbeing and community

  • You will contribute to transforming libraries into community prevention hubs, supporting improved population health outcomes.
