About the Role:As a Case Worker, you’ll work directly with individuals experiencing multiple and complex needs, providing tailored support through a structured case management approach.You’ll empower clients to develop the skills, confidence, and resilience needed to sustain their tenancies and move towards independent living.
Key Responsibilities:
Deliver 1:1 and group-based support using a case management approach
Complete comprehensive risk assessments, support plans, and safety plans
Motivate and empower clients to achieve personal goals
Support development of coping strategies and positive behaviours
Design and deliver engaging activity programmes focused on independence and wellbeing
Encourage client involvement in shaping services and activities
Participate in reflective practice and fortnightly case reviews
Maintain accurate records in line with data protection standards
Monitor tenancy compliance and address any issues appropriately
Build strong partnerships with external agencies
Use motivational interviewing and assertive engagement techniques
What We’re Looking For:
We’re seeking candidates who bring both experience and empathy, with:
Proven experience supporting individuals with multiple complex needs
Ability to engage “hard to reach” clients
Strong relationship-building and communication skills
Experience with risk assessment and support planning
Knowledge in areas such as mental health, substance misuse, domestic violence, brain injury, or welfare benefits
Understanding of Psychologically Informed Environments (PIE) and Trauma-Informed Practice (TIP)
Experience of lone working and managing risk
IT literacy for case recording and reporting
A flexible, proactive approach
🚗 Essential: Full UK driving licence and access to a roadworthy vehicle
