To provide person-centred support to individuals with mental health needs in their homes and the community, promoting independence, wellbeing, and recovery.
Key Responsibilities
* Provide practical and emotional support to service users.
* Support individuals with daily living activities, appointments, and community access.
* Encourage independence, social inclusion, and positive mental wellbeing.
* Monitor and report any changes in a service user's condition.
* Maintain accurate records and follow care plans.
* Work in line with safeguarding, health and safety, and confidentiality policies.
Essential Requirements
* Experience in health and social care or mental health support.
* Compassionate, reliable, and person-centred approach.
* Good communication and teamwork skills.
* Ability to work independently.
* Full UK driving licence and access to a reliable vehicle (essential).
* Enhanced DBS check.
Desirable
* NVQ Level 2 or 3 in Health and Social Care.
* Experience supporting people with mental health conditions Only candidates based in UK and eligible to work in UK are allowed
