An exciting opportunity has arisen for an experienced Registered Nurse to join a leading provider of specialist complex care services, supporting adults with highly complex clinical needs across Yorkshire, the Humber and the North East. You'll play a pivotal role in overseeing complex care packages whilst supporting operational excellence and driving high-quality clinical outcomes.
The Role;
As Complex Clinical Case Manager, you will oversee a portfolio of complex homecare packages, working closely with clients, families and multidisciplinary professionals to ensure safe, person-centred care is delivered at all times. You'll provide clinical leadership across the region, supporting service growth whilst maintaining the highest standards of governance, compliance and workforce development.
Key Responsibilities:
Oversee the clinical and operational management of complex care packages Complete comprehensive client assessments and develop person-centred care plans Carry out competency assessments and clinical supervision of care staff Lead recruitment, staffing and workforce development Ensure compliance with CQC regulations and clinical governance standards Build strong relationships with clients, families and healthcare professionals Deliver specialist clinical training and ongoing support to care teams Support business growth across existing and new complex care packages
The Ideal Candidate:
Active NMC PIN (Adult, Mental Health or Learning Disability Nurse) Previous community care or complex homecare experience Excellent communication skills with the ability to explain clinical information clearly to non-clinical audiences Strong understanding of clinical governance and risk management Experience supporting clients with complex clinical interventions such as: Tracheostomy care Suctioning Non-invasive ventilation Spinal injuries Confident working independently and making clinical decisions Passionate about delivering outstanding person-centred care
