The Role: Discharge Co-ordinator Working closely with NHS hospital staff, youll help to facilitate the flow and discharge of patients from hospitals into our community teams. On a day-to-day basis, youll: Oversee holistic clinical assessments for patients and potential patients, determining suitability for transition into a community setting, including our virtual ward and early supported discharge pathways Co-ordinate discharge plans, identifying and resolving any internal delays Identify care needs and provide support and advise as appropriate Carry out face-to-face reviews of patients medical histories Liaise closely with the Trust MDT and deliver real-time documentation. ? have the confidence to work autonomously in the community and share our commitment to delivering quality patient care. Compassionate and collaborative, youll bring with you strong communication skills and the ability to build effective relationships with patients, carers and colleagues alike. Youll also need: The right to work in the UKwithout any form of sponsorship A drivers licence and access to a car, this is essential as its a mobile role Knowledge of relevant and current legal frameworks and duties relating to hospital discharge and legislation.
