Family Coordinator

We are supporting a client in the Bridlington area who is looking for a dedicated Family Coordinator.

This role focuses on supporting individuals and their families, ensuring they receive the right guidance, communication, and care throughout their journey.

Key Responsibilities:

Providing family support by acting as the main point of contact and offering guidance throughout the support process.
Coordinating communication between families, professionals, and services to ensure everyone stays informed and aligned.
Supporting care planning by helping to create, review, and update plans that meet the individual's needs.
Monitoring progress and reporting any changes, concerns, or updates to the relevant teams.
Helping resolve issues by identifying challenges and guiding families towards the right support or solutions.
Key Requirements:

Experience in family support, social care, or a similar community-based role.
Strong communication and organisational skills.
Ability to build trust while maintaining professional boundaries.
A caring, patient, and proactive approach.
Confidence working independently and as part of a wider team.
Contract Type: Temporary (3-6 months with expectation of an extension)
Pay Rate: £15.12
Working pattern - 37 hours per week, onsite
