Salary: £32,070 - £34,359 per annum (pro rata for part-time) (Pay award pending)


Working Hours: 2 posts x 37 hours per week each (East)


Contract Type:  Secondment - 12 months

About the Role


The HART service provides several care and support offers under its registration as a domiciliary care service. This includes, reablement, which helps people return home from hospital, or prevent them from being admitted to hospital, and regain, their independence. Homecare support to facilitate the timely discharge from acute and community hospitals to better understand the needs of the person once they have been able to return home. Short term crisis support for those experiencing an urgent social care need. These service offers are all short-term support, helping someone to become as independent as possible, and to ensure that the right support pathway is in place to meet any ongoing care and support needs.


Key Duties:


The purpose of the job is to:



  • Be part of a team that acts as the first point of contact for all internal and external customers to the HART service.

  • Accept initial referrals (including out of hours) into the service.

  • Triage and risk assess referrals against the HART service criteria and transfer to appropriate pathway for Duty Officers & Programmers to allocate work.

  • To provide advice and guidance for all referrals which do not meet the criteria for a direct care response.

  • Prioritise urgent referrals to meet key performance indication response times within a two-hour window.

  • Allocate and co-ordinate work with appropriate response. Provide handover to the next shift and escalate any issues or concerns to the Senior Coordinators or out-of-hours HART Manager as appropriate.

  • Embed collaborative working with health partners and other stakeholder.

  • Complete onward referrals to care pathway teams as appropriate and necessary.

  • Provide advice and guidance to other Referral, Duty & Programming and Service Contact and Triage staff as appropriate.


It is an important role in the day to day running of the service which operates 365 days of the year from 7am to 10:30pm with a rolling rota shift pattern which includes regular weekends, early, late and day shifts and bank holidays. 


A flexible approach to work, access to a vehicle and drivers’ licence and an Enhanced DBS is required.

About You:



  • Level 3 Diploma in Health and Social Care or substantial social care experience.

  • Effective planning, managing and prioritising working and meeting deadlines.

  • Experience of working with other agencies and/or working in a multi-agency environment to identify and meet needs with creative and pragmatic solutions.

  • Ability to continuously update knowledge, implement and introduce systems to evidence adherence to legislative/regulatory standards and service frameworks relevant to the service. 

  • Be able to effectively manage, motivate, lead staff and develop trust within the team and encourage team members to build positive, trusting relationships with each other, across departments and with customers, service users, and partners.

  • To liaise with service users and their families regarding any package of care offers following receipt of communication through the brokerage team as appropriate.

  • To support the development of systems, processes and procedures under the direction of the HART Manager to ensure services maintain comprehensive end to end processes. To use Information Technology applications e.g. Microsoft Outlook, Word, Excel and database applications.
