Job Description - Health Link Worker






Overview






Change Grow Live are a charity dedicated to the belief that we can make a difference to our Service Users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.

 

Be Smoke Free is a specialist nurse-led tobacco addiction service delivered by Change Grow Live, in partnership with Manchester Health Care Commissioning Service and Manchester City Council. Our aim is to raise awareness of the harmful effects of smoking while promoting wellbeing across workplaces, community settings, and organisations throughout Manchester.

Working within the smoking service, as a Health Link Worker, you will play a key role in improving access to stop smoking support across the city. You will manage your own caseload, developing it through strong and proactive relationships with key stakeholders. This includes GPs, district nursing teams, asthma and COPD clinics, lung health and diabetes services, as well as teams supporting older people, individuals with long-term conditions, and those experiencing mental health challenges.

 

You will work collaboratively with these partners to identify and engage individuals who would benefit from stop smoking support, ensuring timely referrals into the service. You will then deliver high-quality, tailored interventions—ranging from brief advice and signposting to structured stop smoking programmes—to support individuals to quit tobacco and improve their overall health and wellbeing.

 

Interventions will be delivered flexibly to meet individual needs, including face-to-face support in community settings, remote delivery, and both one-to-one and group-based sessions.

 

Location: Manchester
Hours: Full time, 37.5 hours per week
Contract Type: Fixed Term Contract of 12 months

 

Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, salary and payments will be pro rata based on contracted hours.









Responsibilities






About the Role:

  • Promote the benefits of stopping smoking and the service through a variety of outreach methods, venues, and community events.
  • Pro-actively build relationships with healthcare professionals and community partners to generate referrals and develop a sustainable caseload.
  • Work as part of the specialist team to deliver both brief interventions and structured stop smoking programmes to a diverse population.
  • Deliver interventions in line with NRT, Varenicline, and e-cigarette protocols.
  • Support key workstreams to ensure services are accessible, inclusive, and targeted to priority populations.
  • Engage with professionals and the wider community to raise awareness of smoking-related harm and encourage appropriate referrals into the service.
  • Accurately record all interventions using electronic record management systems.
  • Keep up to date with local and national policy developments relating to smoking cessation.
  • Provide support, mentoring, and training to apprentices, volunteers, students, and new team members as part of their development.
  • Contribute to service delivery by supporting the Nurse Manager in achieving performance targets.
  • Deliver training to new Stop Smoking Advisers and refresher training for existing staff.
  • Provide ongoing mentorship and specialist advice to both internal colleagues and external professionals seeking guidance on stop smoking support.
  • Undertake additional duties as required to support service objectives and outcomes.

About You

We are looking for someone who is passionate about improving health outcomes and confident in building relationships across a wide range of services and communities.

You will have:

  • Experience working within a health and/or social care environment.
  • A good understanding of the harmful effects of smoking and the benefits of quitting.
  • Knowledge of national smoking cessation policies and how they influence service delivery.
  • An understanding of the treatments and interventions available to support smoking cessation.
  • Experience of assessing individual needs and delivering tailored interventions.
  • Strong communication and engagement skills, with the ability to build effective partnerships and motivate behaviour change.
  • Experience of managing risk and maintaining high-quality, safe practice.
  • The ability to maintain accurate and timely records using electronic systems.
  • A commitment to continuous professional development and keeping up to date with emerging trends and best practice.
  • Competence in using Microsoft Office and electronic recording systems.
