Customer First is looking for motivated and dedicated Independence and Wellbeing Facilitators to join our growing team.

Your role and responsibilities
As an Independence and Wellbeing Facilitator, you will play a key role in supporting customers to maintain their independence and wellbeing. You will identify the most appropriate pathway for each customer's journey, provide advice and guidance, and connect people with the support and services they need.

Customer First and the Intervention and Prevention Team are committed to delivering a high-quality service that supports people to live independently and achieve positive outcomes.
