Your role and responsibilities  

• Support the Home First team with customer assessments and reviews.
• Work with allocated customers to identify and deliver high-quality, person-centred outcomes.
• Build strong relationships with customers to achieve personalised goals and improve independence.
• Work collaboratively with social work teams, health partners and the voluntary sector to support integrated care.
• Make a meaningful difference to the lives of residents across Suffolk.
• Receive regular support and professional supervision as part of a collaborative team environment.

You will need

• Strong communication skills when working with customers, their families or advocates, and professional partners.
• The ability and energy to respond to urgent work as part of a team system.
• Resilience under pressure and the ability to remain positive when facing challenges.
