Leaving Care Worker

• Coordinate individual plans and resources from multi-agency partners to ensure young people transition to adulthood safely.
  
  


• Undertake direct work with young people, their peers, and families to achieve strategic outcomes in line with the Children in Care and Care Leavers Strategy.
  
  


• Support and challenge agencies and families to drive lasting change and improve outcomes for care leavers.
  
  


• Engage and build meaningful connections with hard-to-reach young people to enhance their life chances.
  
  


• Support young people to secure education, training, employment, and stable accommodation.
  
  


• Deliver targeted interventions to young people at elevated risk (e.g., anti-social behaviour, poor mental health, or risk of losing accommodation/employment).
  
  


• Participate in a team duty service to support young people in crisis, offering immediate needs-based information, advice, and guidance.
  
  

Experience

• Experience working directly with young people, care leavers, and their wider support networks.
  
  


• Experience delivering targeted interventions to vulnerable individuals or those at elevated risk.
  
  


• Experience creatively engaging hard-to-reach service users to build trust and drive positive life changes.
  
  

Qualifications

• Driving licence and access to a car.
  
  


• Social Work England Registration.
  
  

Additional Information

• Working hours: 37 hours per week.
  
  


• Enhanced DBS required Children's & Adults' Barred Lists.
  
  


• This is a hybrid role (Min 2 days in the office, 3 days on visits or home working) based in Peterborough.
  
  


• We work on a biweekly payment schedule.
