Job Description - Lifelong Links Coordinator







2 Month Contract With A Local Authority







Job Purpose


The Lifelong Links Coordinator works directly with children and young people to help them build and strengthen lifelong support networks. The role focuses on identifying, engaging, and bringing together family members, friends, and other important people to develop sustainable support plans that improve long-term outcomes. Working closely with Children's Social Care and partner services, the Coordinator ensures the child's voice remains central throughout the Lifelong Links process while promoting safe, strengths-based, and relationship-focused practice.














Key Responsibilities



  • Manage a caseload of children and young people referred to the Lifelong Links service.

  • Build trusting relationships with children and young people to understand their needs, wishes, and experiences.

  • Identify, map, and engage the child's wider support network, including family members, friends, professionals, and significant past connections.

  • Coordinate and facilitate Lifelong Links meetings, ensuring the child's voice is at the centre of all planning.

  • Develop practical, sustainable support plans alongside children, young people, and their networks.

  • Work collaboratively with Children's Social Care, Family Group Conference, Parent Advocacy, and partner agencies to achieve positive outcomes.

  • Maintain accurate, timely case records using MOSAIC and other relevant systems.

  • Promote the principles of Lifelong Links while adhering to safeguarding procedures, professional standards, and organisational values.










Requirements



  • Completed Lifelong Links Coordinator training with experience applying the model in practice.

  • Experience working directly with children, young people, and families, including those with complex needs or safeguarding concerns.

  • Ability to build trusting relationships and engage individuals from diverse backgrounds using a culturally sensitive, anti-oppressive approach.

  • Strong organisational skills with the ability to manage a caseload independently and prioritise effectively.

  • Experience facilitating meetings, managing conflict, and supporting collaborative decision-making.

  • Knowledge of safeguarding legislation and statutory responsibilities relating to children and young people.

  • Confident using family mapping tools, electronic case management systems (including MOSAIC), and maintaining accurate records.

  • Enhanced DBS check required.

  • Location: Southwark (London)

  • Hours: Full-time (36 hours per week)

  • Pay: Bi-weekly payments
