Job Description - Mental Health Social Worker

A Local Authority mental health social work team is seeking an experienced Social Worker to work alongside health colleagues within a Community Mental Health Team based at Dewi Sant Health Park, Rhondda Cynon Taf.


What will your responsibilities be? 



  • Assess and support citizens within a mental health setting

  • Work collaboratively with multidisciplinary health teams in a CMHT

  • Complete Social Services and Well-being (Wales) Act assessments and support planning

  • Carry out Best Interest (BI) decisions and Mental Capacity Act (MCA) work

  • Lead on safeguarding enquiries and interventions

  • Provide duty cover as required

  • Attend Quality Assurance panels for funding provisions

  • Support effective hospital discharge processes and community transitions


Qualifications and Experience: 


To be successful in this role you must have;  



  • Proven experience within mental health or CMHT settings

  • Strong working knowledge of the Social Services and Well-being (Wales) Act, MCA, and safeguarding frameworks

  • Ability to manage a complex caseload in a fast-paced environment

  • Excellent assessment, communication and decision-making skills

  • Available to start immediately
