Our North & Mid Outreach team operate throughout the Burgess Hill/Haywards Heath/Crawley/East Grinstead areas so being a car driver is essential for this role.

About the RoleAs part of our outreach team, you will:

Provide personalised, community-based support to individuals with complex mental health needs, promoting independence within their own homes.
Work closely with clients to implement and review tailored care plans, ensuring their safety and well-being.
Collaborate with multidisciplinary teams, including clinical professionals, to deliver trauma-informed and recovery-focused care.
Maintain accurate records and report any changes in client condition or concerns promptly.
Participate in ongoing training to develop skills in mental health support and person-centred approaches.
Essential qualifications include strong communication and interpersonal skills, empathy, reliability, and a willingness to learn and work flexibly within a community setting.
Previous experience in social care or mental health support is desirable but not essential.

Requirements

We are seeking Support Workers who are:

Strong communication and interpersonal skills to effectively engage with individuals and multidisciplinary teams.
Empathy and a genuine commitment to supporting people with complex mental health needs.
Reliability and a professional attitude, including punctuality and accountability.
Willingness to learn and adapt within a flexible, community-based work environment.
Ability to work independently and as part of a team, maintaining a person-centred approach.
Basic understanding of mental health, trauma-informed care, and recovery principles is advantageous.
Competence in maintaining accurate records and reporting changes in client condition promptly.
Commitment to ongoing professional development and training to enhance skills and knowledge.

Essential:

Car owner/driver is essential for this role.

Desirable:

Experience supporting individuals with mental health conditions in community settings
Only candidates based in UK and eligible to work in UK are allowed
