What We’re Looking For:

• Proven Care Coordinator experience within domiciliary care


• Ability to manage rotas and respond quickly to service changes


• Level 3 in Health and Social Care (or equivalent)


• Excellent communication, organisation, and time management skills


• Confident working independently and making informed decisions

Key Responsibilities:

• Plan, organise, and maintain weekly rotas for care staff


• Coordinate care visits to ensure timely, safe, and consistent support


• Build strong relationships with carers, clients, and external professionals


• Respond to day-to-day changes, ensuring continuity of care


• Support care workers with queries, updates, and service delivery needs


• Maintain accurate records and uphold compliance standards
