Adult Social Worker Opportunities - Norfolk County Council (Hospital Discharge Roles £44.65ph)

We are currently recruiting for two experienced Adult Social Workers to join Norfolk County Council hospital discharge services, based at:

Norfolk and Norwich University Hospital (NNUH), NR4 7UY - Acute Hospital Discharge Team
Willow Therapy Unit, Norwich Community Hospital, NR2 3TU - Community Hospital / Rehab Discharge TeamBoth roles are available for an immediate start on a 6-month contract.

These are exciting opportunities to work within fast-paced multidisciplinary hospital environments, supporting safe and timely discharge through a Home First approach, promoting independence, recovery, and person-centred care.

Key Responsibilities:

Undertake Care Act 2014 assessments and support planning
Complete Mental Capacity Assessments and Best Interest decisions
Support safe and timely hospital discharge planning
Work closely with health professionals, MDTs, and families
Undertake Continuing Healthcare (CHC) reviews
Promote independence and reduce delayed discharges

Requirements:

Qualified Social Worker registered with Social Work England
Experience in hospital discharge or community adult social care
Strong knowledge of Care Act 2014, MCA 2005, and safeguarding adults
Ability to work in a fast-paced acute or community hospital setting
Excellent communication and MDT working skills

Rate: £44.65 per hour
