Nurse Case Manager – Autism Manchester | £47,000 FTE | 15 Hours Per Week | 2 Days | Hybrid
Are you a Registered Nurse with experience supporting autistic adults and looking for a flexible, specialist role outside of traditional nursing?
We’re recruiting for a Nurse Case Manager to provide clinical oversight across supported living and community services in Manchester, supporting autistic adults to live safely, independently and with greater choice.
This is a part-time position working 15 hours across two days per week, with one fixed day and flexibility around your second working day.
The Role
As Nurse Case Manager, you’ll take responsibility for the clinical oversight of individuals receiving support across supported living and community settings. You’ll:
Complete comprehensive clinical assessments and reviews Develop and update person-centred care plans and risk assessments Provide specialist nursing input for autistic service users Support individuals with complex health, behavioural and communication needs Provide clinical guidance and support to care teams Work closely with families, carers and multidisciplinary professionals Support safeguarding, clinical governance and risk managementWhat We're Looking For
We’re looking for an experienced Registered Nurse with:
Current NMC registration Strong experience working with autism Experience within learning disabilities, mental health, complex care, supported living or community services Confident assessment and care planning skills Good knowledge of safeguarding and clinical risk Strong communication and multidisciplinary working skills Ability to work autonomously across community-based servicesExperience with Positive Behaviour Support (PBS), behaviours of concern, complex autism or learning disabilities would be highly advantageous.
