7 months Contract with a Local Authority

Job Purpose

As an Occupational Therapist within the Regaining Independence Service (RIS), you will support adults to maximise their independence and achieve their personal goals through community-based occupational therapy interventions. Working within customers' own homes, you will undertake assessments, develop person-centred support plans, and deliver specialist interventions to improve outcomes and quality of life.

Key Duties/Accountabilities

• Undertake comprehensive Occupational Therapy assessments within the community.
  
  


• Support customers to maximise independence and remain safely in their own homes.
  
  


• Establish meaningful and achievable goals with service users.
  
  


• Deliver specialist Occupational Therapy interventions.
  
  


• Develop, implement, and review support and rehabilitation plans.
  
  


• Monitor progress and adapt interventions to achieve the best possible outcomes.
  
  


• Work closely with reablement providers and multidisciplinary teams.
  
  


• Provide professional advice and recommendations regarding independence and daily living activities.
  
  


• Maintain accurate case records and assessments.
  
  


• Apply evidence-based Occupational Therapy practice.
  
  


• Manage a varied caseload across the North and West areas of West Sussex.
  
  


• Work collaboratively with health and social care professionals.
  
  


• Promote person-centred practice and positive outcomes for service users.
  
  

Essential Experience Required

• Qualified Occupational Therapist with recent adult social care experience.
  
  


• Experience completing Occupational Therapy assessments.
  
  


• Experience within Reablement, Rehabilitation, Community Services, Hospital Discharge, Adult Social Care, or Independence Services.
  
  


• Experience developing and reviewing care and support plans.
  
  


• Experience working with adults with a range of physical, cognitive, and functional needs.
  
  


• Strong assessment, analytical, and problem-solving skills.
  
  


• Experience working within multidisciplinary teams.
  
  


• Experience managing a caseload independently.
  
  


• Excellent report writing and record-keeping skills.
  
  


• Good IT and communication skills.
  
  

Essential Qualifications Required

• Degree or Diploma in Occupational Therapy.
  
  


• HCPC Registration.
  
  


• Enhanced DBS.
  
  


• Right to Work in the UK.
  
  

Additional Information to Note

• Working hours: 37 hours per week.
  
  


• Monday to Friday, 09:00 - 17:00.
  
  


• Location: County Hall North, Chart Way, Horsham, RH12 1XL.
  
  


• Hybrid working arrangement with at least 1 day per week office attendance.
  
  


• Grade 10.
  
  


• Enhanced DBS required.
  
  


• HCPC Registration required.
  
  


• 5 years referencing and full employment history required.
  
  
  


• Interview required.
