Job Description - Occupational Therapist - Home first Team
Your new company Our Home First teams are at the forefront of transforming adult social care and hospital discharge services across Oxfordshire. The successful Occupational Therapist will be based within the Home First South Team, with an office based at Audlett Drive, Abingdon.
As a Home First Occupational Therapist, you will:
Assess individuals in their own homes following discharge from hospital. Support safe and timely hospital discharge through the Discharge to Assess pathway. Complete functional assessments and activity analysis. Recommend and prescribe equipment, assistive technology and minor adaptations. Undertake moving and handling assessments and reviews. Work collaboratively with multidisciplinary professionals to develop personalised support plans. Promote independence and reduce reliance on ongoing care, where appropriate. Support individuals, families and carers through periods of change and recovery. Maintain accurate and timely case records.What you'll need to succeed
A qualified Occupational Therapist registered with the HCPC. Passionate about enabling people to live as independently as possible. Skilled in assessment, problem-solving and clinical reasoning. Experienced in equipment provision, moving and handling, or community rehabilitation. Able to work autonomously while contributing positively to a multidisciplinary team. Committed to strengths-based practice and the principles of the Oxfordshire Way. A confident communicator with excellent organisational skills.
