Occupational Therapist Reablement Service

Our client is recruiting for an Occupational Therapist to join the team at Craven Vale, a vital 16-bed reablement service that plays a key role in supporting hospital discharges and preventing unnecessary hospital admissions across Brighton.

This is an opportunity to work within a highly rewarding service where your expertise will directly support individuals to regain independence, achieve positive outcomes, and return safely to their own homes.

The Role:

As the Occupational Therapist within the service, you will be central to the delivery of high-quality reablement interventions, working with individuals transitioning from hospital and those requiring community-based support to avoid admission.

Your assessments and interventions will help maximise independence, reduce long-term care needs, and facilitate safe and effective discharge planning.

What will your responsibilities be? 

• Undertake Occupational Therapy assessments within a reablement setting.


• Develop and implement person-centred intervention plans.


• Assess functional ability and identify opportunities to maximise independence.


• Recommend and arrange appropriate equipment and adaptations.


• Support timely and effective hospital discharges.


• Work collaboratively with multidisciplinary teams, care staff, and healthcare professionals.


• Contribute to preventing avoidable hospital admissions through proactive assessment and intervention.


• Maintain accurate and timely clinical records.

Qualifications & Experience: 

• Degree in Occupational Therapy and HCPC registration


• Experience working within reablement, community, hospital discharge, or intermediate care services


• Strong assessment and rehabilitation skills


• Knowledge of equipment provision and functional assessments


• Excellent communication and multidisciplinary working abilities


• Ability to work independently and manage a varied caseload
