Ensure safe and smooth discharge of patients from the hospital.
Collaborate with patients, families, and healthcare professionals to coordinate post-discharge services.
Develop and implement appropriate care plans tailored to individual patient needs.
Act as a liaison between hospital staff and external care providers to ensure continuity of care.
Address any challenges or concerns to facilitate a seamless transition for patients.
