Job Description - Placement and Brokerage Officer

3 months contract with a local authority

Summary

The Placement & Brokerage Officer is a pivotal member of the Care Arrangement Team, tasked with coordinating and arranging care packages for adults with assessed care and support needs. This role involves close collaboration with social care professionals, health partners, care providers, and service users to ensure the provision of timely, person-centred, and cost-effective support. The position is a 3-month contract with a local authority, offering a unique opportunity to make a significant impact on the lives of individuals requiring care.

Responsibilities

Arrange and broker care packages for individuals with assessed care and support needs.

Record care arrangements accurately within designated case management systems.

Manage and prioritise referrals, ensuring urgent cases, including hospital discharges, are processed efficiently.

Source suitable providers in line with individual assessed needs and organisational requirements.

Coordinate out-of-area placements, ensuring continuity of care and compliance with relevant procedures.

Ensure brokerage activity remains within agreed financial and contractual parameters.

Escalate exceptions, service risks, and placement challenges appropriately.

Requirements

Experience working within an Adult Social Care environment.

Experience coordinating services, placements, referrals, or care packages.

Experience working with multiple stakeholders, including care providers, health professionals, and service users.

GCSEs (or equivalent), including Mathematics and English.

Additional Information

Working hours: 36 hours per week

Location: 2 Alice Square, Birkenhead, Merseyside, CH412YA, United Kingdom

Work pattern: 2 days remote, 3 days in the office

Payment schedule: Bi-weekly
