Job Description - Preparing for Adulthood Social Worker

We’re looking for an experienced Social Worker to join a Preparing for Adulthood team. You’ll support young people with additional needs and disabilities through the transition to adult services, helping them and their families plan for the future with confidence.

You will work with young people approaching adulthood, their families and multi-disciplinary partners to complete assessments, develop person-centred plans and coordinate the support needed for a successful transition. This will include supporting planning around education, employment, health, housing, independence and community inclusion.
What you’ll need
A recognised Social Work qualification Current Social Work England registration Experience supporting young people with disabilities, additional needs or complex needs Knowledge of transition planning and preparing for adulthood pathways Strong assessment, care planning and partnership-working skills A person-centred, strengths-based approach to practice Why work with Leaders in Care?
