A values‑driven organisation is seeking a Referrals & Development Lead/Manager to support the growth of specialist supported living services for people with learning disabilities, autistic people and individuals with complex needs throughout Yorkshire.

You’ll oversee the full referral process — from initial enquiry to assessment, transition and placement — ensuring every move is safe, person‑centred and well‑coordinated. You’ll also contribute to wider service development, including feasibility assessments, business cases and the opening of new supported living services.

Key Responsibilities

* Manage referrals from enquiry through to placement

* Coordinate assessments, visits and transition planning

* Work closely with operational and clinical teams

* Build relationships with Local Authorities, ICBs and housing partners

* Support new service development and openings

* Complete feasibility assessments and business cases

* Ensure new developments meet CQC, NICE and regulatory standards

* Contribute to quality, governance and operational planning

About You

* Strong experience in Health & Social Care referrals or service development

* Skilled in working with commissioners and MDTs

* Knowledge of supported living and complex transitions

* Excellent organisation, communication and stakeholder engagement

* Confident decision‑maker with a person‑centred approach

* Level 5 qualification desirable

* Full UK driving licence
